2 Introductions for filling in the application form

2.1 “Certified products” in the cover is the “product category” defined in the implementation rules-clause 4.1.1 , such as bicycles for young children, electrically driven ride-on vehicles for children, electrically driven toys, video toys, static plastic toys and so on.
2.2 “Applicant” indicates the organization applying for certification, which is also the certificate holder. The relevant information should be filled in according to the business license.
2.3 “Manufacturer” is usually the brand holder of the certified products, and the first responsible organization for product quality regulated in the Product Quality Law of the People’s Republic of China . The registered address, post code and the legal status in the column should be filled in according to the business license.
2.4 “Factory” is the plant making the final products. The factory address and the post code indicate the address and postcode of the actual manufacturing premises of the certified products.

Notes: Contract or agreement among the applicant, the manufacturer and factory should be submitted following the application form, in the case that the applicant, the manufacturer and factory are different.

2.5 “General information about the product”

Instructions: model means the code name, which corresponds exclusively to each toy’s function, construction, appearance, dimensions, color and so on.

Model should corresponds to the photos of the products under application.

We take the electric toys and static plastic toys as examples to show you how to fill in the “General information about the product” form.

2.6 The “Agent” is the organization helping the applicant to apply for the certification. If applicant applies for the certification oneself, the relevant information about the agent is not needed.
2.7 A signature and stamp are needed in the appropriate place of the application form, the agreement and the declaration of conformity.

3 Instructions for the materials need to be submitted

3.1 “The registration license of the applicant and/or factory” should be the copies of business license after annual inspection.
3.2 “General information about the factory” includes production scale, capability and history of the products under application.
3.3As for “The critical production equipment list of the factory”, “The list of the main testing equipment and instrument of the factory” and “The list of the critical components and the main materials”, the contents needed should be filled in according to the actual situation of enterprise and meet the requirements of relevant implementation rules. You can download the form “The list of the production and testing equipment” as a reference

The contents needed in “The critical production equipment list of the factory” can be obtained form the nameplate or the instructions of the production equipment, or from the relevant documents of the enterprise.

The contents needed in “The list of the main testing equipment and instrument of the factory” can be obtained from the packaging, instructions or the calibration certificate of the testing equipment, or from the relevant documents of the enterprise.

The contents needed in “ The list of the critical components and the main materials” can be obtained from the packaging or instructions of the critical components and the main materials, or from the relevant documents of the enterprise.

3.4 The applicant can download the form “The relevant information of the products” as a reference.
3.5 “Photos or pictures sufficient to identify the major characteristics of the products under application” should includes the contents below:
- The toys should be taken out from the packaging, and the toy, the packaging, the accessories in the packaging, such as the charger, battery and the changeable parts should be included in the photos or pictures, and the packaging should have clear Chinese instructions.
- If some toys have doors that can be opened or ropes that can be pulled, these information should be indicated in the photos or the pictures.
- The dimensions of the main body should be indicated in the photos or the pictures, or listed in “The relevant information of the products”.
